REGISTRATION PROCEDURES 1. Select the classes of your choice according to the schedule, staff recommendation, and personal preference. Call the studio (419-874-6773) if you have a question as to which level of dance will be best for your child. 2. Complete the registration form available on line Click Here for Registration Form and return it with the tuition payment for the first eight week pay period and registration fee. No registrations will be accepted without completed registration form and full payment of fees. 3. You may return your registration form and fees by mail to Perrysburg Academy, 156 W. South Boundary, Perrysburg, OH 43551; oryoumay also mail to Perrysburg Gymnastic Center, 12905 Eckel Jct. Rd. Perrysburg, Ohio 43551, or stop at the studio or gym in person. 4. Assume you have been accepted into the classes of your choice unless otherwise notified. 5. You may call the studio 419-874-6773 or gym 419-874-9383 anytime for assistance.
WHAT DO I NEED FOR CLASS? The following attire is required for dancers at Perrysburg Academy Style numbers have been included to give you a large selection. You may purchase your supplies at Dancers Point on Louisiana Avenue (tell them you are from Perrysburg Academy for a discount). You may also order online through Discount Dance Supply - To receive an additional discount, use Dance Teacher ID# TP25599 (www.discountdance.com)
Girls: Leotard of choice with tights, biker shorts, or jazz pants of choice. No baggy sweatpants. Pants must be of a length so the foot can be seen.
Boys: Tee shirt (tucked in) and jazz pants.
Adults: (age 18+): Dress as you wish for maximum comfort and learning. Leotards, etc. are not required for adults.
All Classes: Hair neatly restrained, all jewelry removed (small post earrings permitted), T-shirts, baggy shorts & sweat suits removed.
And lastly...Please be kind to your fellow dancers (and teachers!) and exercise good hygiene habits. Be sure that at the very least hands and faces are washed, that you wear clean socks and underwear, and use deodorant before class.

NOTE: PLEASE!! NO BEDROOM SLIPPERS!!! We realize that Department Stores sell the cutest little leotards for girls, but DO NOT purchase the “made to look like ballet shoes” bedroom slippers that they display with their children’s dancewear. These are bedroom slippers, NOT BALLET SLIPPERS. These shoes are not meant for dance, and they are also more slippery than a standard ballet slipper. Teachers will not allow children into class with bedroom slippers. We thank you for understanding.
BALLET/POINTE Pink slippers Bloch S0203L/S0208L/S0278L Capezio 2027, 205C No canvas slippers Pink pointe shoes. The shape of your foot will determine the type best for you. NO SUEDE TIPS. TAP and Tap combination class You will need Camel tap shoes (Bloch S0302L/S0302G; Capezio 455). Advanced classes will also need Black tap oxfords (Capezio CG100/CG16; Leo's 5068). Heels are not permitted in classes without teacher approval. Adult classes will wear black tap shoes. (Bloch S0302L/S0323L; Capezio 455)

BALLROOM Comfortable attire, smooth soled dress shoes (no flip flops or sandals)
AT NO TIME IS A RECITAL COSTUME APPROPRIATE FOR CLASS WEAR
Dance Bag supplies: Dance Shoes, Band-aids & other first aid items as needed, hair care supplies (hair nets, Bobby pins, comb, brush, ponytail holders, etc), water bottle, a nutritious snack, sanitary needs, extra tights, a good book to read, emergency money, and any other items that will help you make the most of your classes!
